Please come to the surgery and our reception team will be pleased to help you if the form does not allow you to register.
Your registration will be processed in a couple of working days.
In the meantime, if you need medical help, you can contact your previous surgery for advice or visit the Urgent Treatment Centre at Crawley Hospital.

To register with us, please check you live within our catchment area; you can do this using the map below.
- In order to keep our list size to manageable proportions we are only able to accept new patients who live within the Practice catchment area.
Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration’ form.
Newborn Babies

Once notified by the parent/carer or hospital of a birth we send an invitation letter for a 6-8 week check. This letter is sent approximately 7-10 days before the check appointment.
Please note that 6-8 week check invitations are never automatically sent as the practice must receive a notification from the hospital and/or parent/carer of the birth first.

Temporary Patient Registrations
You can register temporarily with a GP surgery while away from home for work, study or on holiday. You’ll remain registered with your permanent GP surgery.

Details of any treatment you have will be passed to your permanent GP surgery. They will add the information to your medical records.
To register as a temporary resident you will need to contact the surgery and complete and return this form.
Safe Surgery
In the UK, we run clinic and advocacy programmes in London that provide medical care, information and practical support to excluded people such as destitute migrants, sex workers and people with no fixed address.

Everyone living in England has the right to free care from a GP.
- Ask reception for an interpreter if you find it difficult to communicate in English.
- Our receptionists won’t ask you about your immigration status.
- If you are worried about giving us your address, please let us know.
- Your information is safe with us.
Overseas Visitors
Those who visit the UK from overseas must pay for their care when they are in England.
In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.
Find Your NHS Number
Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.
This online tool will help you find your number if you do not know it.
Please Note
You do not need to know your NHS number to use NHS services, but it can be useful to have it.
Other ways to get your NHS number
If you cannot get your NHS number online you can:
- find it on any letter from the NHS like a prescription or appointment letter
- call your GP surgery and ask them for your number
