REGISTER WITH THE PRACTICE?
You may register as a patient by calling at our reception where you will be asked to complete a new patient questionnaire.
To register at the practice you will need to provide the following:
• NHS Number
• Proof of ID (e.g. passport/driving licence)
• Proof of residence (e.g. bank statement, utility bill, or other official documentation showing your current address).
As part of the normal registration process you will be registered as a patient of the Practice. You do, however, have the opportunity to request appointments from the doctor of your choice, and we will endeavour to comply with your request if possible.
