New Patient Registration
You may register as a patient by calling at our reception where you will be asked to complete a new patient questionnaire (can be found under the "pre-registration" tab).
In order to register you must be resident in the UK (entitled to be in the Country and living for most of the year within the Practice’s catchment area).
As part of the normal registration process you will be registered as a patient of the Practice. You do, however, have the opportunity to request appointments from the doctor of your choice, and we will endeavour to comply with your request if possible.

They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
PLEASE NOTE THAT SENDING THIS FORM DOES NOT AUTOMATICALLY REGISTER YOU WITH THE SURGERY.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Before your registration is complete, you must fill in and sign the attached form.

Please note that there can be a delay with transferring notes from one surgery to another. This is a national problem which should be resolved shortly.
